About Us

Guiding patients upstream.

Lowcountry healthcare
15+ Years

Lowcountry Primary Care is a health and wellness company born from years of intimate experience caring for the sick and vulnerable. Founded by an accomplished hospitalist, weight loss physician and hospice medical director, Lowcountry Primary Care reflects more than fifteen years spent at the bedside of patients facing serious and life-limiting illness. As an internal medicine physician devoted exclusively to hospital care, the founder has long taken pride in restoring health with skill, compassion, and dignity whenever possible, and in guiding patients and families with clarity and humanity when cure is no longer attainable.

Through this work, a deeper realization emerged: while treating illness is essential, preventing it is transformative. Rather than focusing solely on crisis care, a vision took shape for a practice dedicated to preserving vitality, resilience, and long-term wellness. From that vision, Lowcountry Primary Care was born.

Healthcare philosophy
Our Philosophy

The Parable of the River

Our philosophy is captured by the timeless "Parable of the River," a public health allegory in which rescuers tirelessly pull drowning victims from the water until one rescuer walks upstream to discover the source of the harm, a broken bridge. The lesson is simple yet profound: while rescue is necessary, true impact comes from addressing root causes before crisis occurs. Prevention is not a luxury, it is the highest form of care.

At Lowcountry Primary Care, our mission is to empower individuals with refined, evidence-based tools that support informed choices, restore balance, and promote lasting health. We exist to guide our clients upstream, toward renewal, longevity, and a life lived with intention.

Board Certified Internal Medicine

Daniel R. Hess, MD, MBA

Founding Physician

Dr. Daniel Hess is a board-certified Internal Medicine physician with more than 15 years of experience caring for adults across a broad spectrum of health and illness. Throughout his career, he has remained committed to a straightforward principle: exceptional medical care begins with listening carefully, communicating clearly, and treating every patient as an individual.

Dr. Hess completed his Internal Medicine residency at the University of South Carolina/Palmetto Health, where he served as Chief Resident. Since completing his training in 2010, he has spent much of his career as a hospitalist, caring for patients with both acute illness and complex chronic medical conditions. His experience extends beyond hospital medicine to include weight management, hospice and end-of-life care, quality improvement, and physician leadership.

Board Certified Family Nurse Practitioner

Beth Sanders, MSN, APRN, FNP-BC

Nurse Practitioner

At the heart of Beth Sanders' approach to healthcare is a simple belief: every patient deserves to feel heard, respected, and genuinely cared for. As a board-certified Family Nurse Practitioner, Beth is passionate about providing personalized, relationship-based care that focuses on the whole person, not just a list of symptoms.

Beth believes that great healthcare begins with listening. She takes the time to understand each patient's concerns, goals, and lifestyle, then works alongside them to create a thoughtful and practical plan for their health. Her approach combines evidence-based medicine with education, prevention, and a strong emphasis on the patient-provider relationship.

Services

A Better Way to
Experience Primary Care

At Lowcountry Primary Care, we practice Direct Primary Care (DPC), a membership-based approach to healthcare designed to restore something that has become increasingly difficult to find in traditional medicine: time and access to your healthcare provider.

Unrushed Office Visits

Appointments are intentionally scheduled with more time than is typical in a traditional medical practice. Whether you have an acute concern, need help managing a chronic condition, or simply want to discuss your health, our goal is to give you the time you need without feeling rushed.

24/7 Access to Your Provider

Health questions don't always happen during office hours. Members have direct access to their healthcare provider when they need guidance, including after hours when appropriate.

Patient having blood pressure checked 03

Telemedicine

Not every medical concern requires a trip to the office. Members have access to convenient telemedicine visits when the nature of the problem makes virtual care appropriate.

Annual Screening Laboratory Testing

Your membership includes a set of basic screening laboratory tests each year, selected based on age, medical history, and clinical appropriateness. We don't simply order tests, we review your results with you and help you understand what they mean for your health.

Preventive & Wellness Care

We emphasize prevention, appropriate health screenings, lifestyle counseling, and identifying health risks before they become larger problems.

Chronic Disease Management

For members living with conditions such as high blood pressure, diabetes, high cholesterol, obesity, thyroid disease, and other common medical conditions, we provide ongoing monitoring and individualized treatment.

Acute & Routine Primary Care

From common illnesses and minor injuries to medication management and everyday health concerns, members have a trusted healthcare team to turn to when something comes up.

What Direct Primary Care Is, And What It Isn't

Understanding how membership-based healthcare works

Direct Primary Care is not health insurance, and membership in Lowcountry Primary Care is not intended to replace comprehensive medical insurance.

Your membership covers the primary care services provided by our practice as described in your membership agreement. It does not cover services provided outside of Lowcountry Primary Care, such as hospitalization, emergency department care, specialist care, surgery, advanced imaging, or medications and testing obtained from outside facilities.

For this reason, we encourage our members to maintain appropriate health insurance or other coverage for significant and unexpected medical expenses.

DPC + High-Deductible Insurance

Many people choose to pair Direct Primary Care with a high-deductible health plan or other coverage designed primarily to protect against major medical expenses. In this model, your Lowcountry Primary Care membership provides accessible, relationship-based primary care for everyday healthcare needs, while your insurance remains available for expenses such as hospitalization, surgery, specialty care, advanced testing, and emergencies.

The right insurance arrangement varies from person to person, so we encourage patients to choose coverage appropriate for their individual and family needs.

Additional Services

Additional Health & Wellness Services

In addition to primary care, Lowcountry Primary Care plans to offer select health, wellness, and aesthetic services for members who may benefit from them.

These services are not included in the monthly membership fee and are billed separately.

Botox®

Botulinum toxin treatments may be available for both cosmetic purposes and selected medical conditions, including appropriate patients with conditions such as temporomandibular joint (TMJ) symptoms or certain headache disorders.

Advanced Wellness Laboratory Testing

For patients interested in looking beyond routine annual screening, additional laboratory testing may be available based on individual health goals, risk factors, symptoms, and medical history.

Wellness Infusions & Injectable Therapies

Select vitamin, nutrient, and other wellness therapies may be available, including certain peptide-based therapies when clinically appropriate.

Our Promise

Primary Care Built Around You

We believe good medicine requires more than ordering tests and prescribing medications. It requires knowing your history, understanding your goals, answering your questions, and being available when you need help.

Your membership gives us something increasingly rare in healthcare: the time to do that well.

For the first time, I feel like my provider actually has time to listen, not just to my symptoms, but to me.

Common Questions

Everything you might be wondering.

New to direct primary care? These are the questions every first-time member asks.

Direct primary care is a membership model of medicine. You pay a flat monthly fee directly to your provider, and in return you get unhurried visits, same or next-day access, direct communication, and transparent pricing, no insurance claims, no copays, no surprise bills.
No. A membership covers primary care, not hospitalizations, specialists, or emergencies. Most members pair their membership with a high-deductible health plan to cover major events and specialized care.
Yes. Beginning in 2026, qualifying DPC membership fees may be paid tax-free using HSA funds for eligible individuals. Our individual membership rates are structured within the applicable 2026 federal DPC limits. Individual HSA eligibility varies. Please consult your tax or benefits professional regarding your specific circumstances.
Your provider coordinates referrals, communicates directly with your specialist, and stays involved in your care. Costs outside the office, specialists, imaging, hospitalization, are handled through your separate insurance, and we'll help you navigate them.
Members have 24/7 direct access. You can text or call your provider for guidance day or night, and urgent concerns are answered promptly, often avoiding an unnecessary trip to urgent care or the ER.
Labs and imaging are billed at cost, exactly what the lab charges, with no markup. Your provider reviews every order and discusses pricing with you before anything is run, so there are no surprises.
Yes. Membership is month-to-month with no long-term contract. Cancel anytime with 30 days' notice, no exit fees, no phone calls, no hassle.
Adults who want a real, ongoing relationship with one provider, including those with high-deductible plans, the uninsured, and the underinsured. If you're tired of ten-minute appointments, this is built for you.
